Please complete all sections and submit the Consent to Release Confidential Information from Koʻolauloa Health Center form. Please note, that you and a witness must sign the form or we will not be able to process your request.

The request may take up to 30 days to fulfill. Requests for electronic records may take up to 7 days to fulfill. If you are requesting medical and behavioral health records, separate forms must be used.

Koʻolauloa Health Center is a Federally Qualified Health Center (FQHC), is deemed by the Health Resources and Services Administration (HRSA) to be a federal employee for purposes of medical malpractice claims and, as such, qualified for protection under the Federal Tort Act (FTCA).

This health center receives U.S. Department of Health and Human Services (HHS) funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
